Legal Forms

Generally, every time we do a big, major event, we will need to do the legal paperwork. The links below are PDF forms from the Diocese, and they should work well for our purposes too:

Youth Consent Form. Every kid who comes with us on events such as bike outings MUST have one of these forms on file! (And do notice that we need a new form for each event!)

Drivers' Declaration Form. Every driver who transports kids as part of an event MUST have one of these forms on file with the church office! (By the way, notice that drivers must be 18+ years old.)
